Each Pokémon can hold one item. A held item has a tier from 1 to 5, a bonus to one stat (HP, Attack or Ability Power) and an effect of its own:

Getting held items
- Won battles drop them sometimes, more often on the boosted map, on gym floors and in raids (where they also come in better tiers).
- An Item Box holds a random held item; a Rare Item Box one of tier 3 or better.
Forging and scrapping
- Forge two held items of the same tier, with some stardust, into one of the next tier.
- Scrap held items you don't need into stardust.
Mega Stones
A Mega Stone is a held item that lets its Pokémon Mega Evolve in battle. It takes the held item slot, so it replaces a held item's bonus. Mega Stones only come from Mega Raids, as rarely as the species turns up shiny, and never one you already have. They can't be forged, scrapped or sold.
